In the original game introduction, Osana behaves like a stereotypical tsundere while talking to Senpai after Ayano Aishi accidentally bumps into him. If Ayano plays as a delinquent, Osana will refuse to follow her, but she can still be convinced to help with other tasks. Osana's romantic interest is Kyuji, and players have the ability to matchmake them in the demo. Ayano expresses her desire to prevent Osana from winning Senpai's heart, even if it means resorting to violence. Info-chan urges Ayano to make Osana suffer. If Ayano tries to take a picture of Osana with her phone, she will become annoyed and Ayano will receive a reputation penalty. Osana also worries about whether Senpai will like the lunch she made for him, but her friend, Raibaru, encourages her and they go back to the plaza. Befriending Osana will cause her to no longer interact with Senpai. In a cutscene, Osana and Senpai are seen having a conversation near one of the bathrooms.
Osana is close friends with Raibaru, another popular student, and they are often seen together, including sunbathing with the Bullies. It is unclear why Osana is able to become good friends with Raibaru despite only meeting her last school year. However, Osana will willingly follow Ayano if she has a high reputation, even if Ayano has only interacted with her briefly. If Ayano takes inappropriate pictures with her phone, Senpai will be disgusted and will walk home alone. If Ayano damages Osana's book, Senpai will be disappointed in Osana and will also walk home alone. If Osana's phone is not stolen, Senpai will praise the pictures and they will walk home together. The rivals are the main antagonists in Yandere Simulator and each one appears in the game every week. Jokichi Yudasei, Ryoba's husband and Ayano's father, is a significant character in the game.
Some of the main characters have not been fully implemented yet, but the mod offers a premium quality experience with handmade stickers printed on high-quality paper. It is important to note that Yandere Simulator is a horror game with a unique twist on the traditional love story theme. The protagonist, Yandere-chan, is madly in love with an upperclassman, showcasing the yandere trope which depicts a character obsessively infatuated with their romantic interest to a dangerous degree.
